How do I use Whole Person Care Summary?

Whole Person Care Summary compiles information from 15 data sources to provide a quick snapshot of a client's current care, risk factors, and service utilization. This section of the user guide will teach you how to use the features on the homepage and help you understand the information in the left-navigation links.

How Does Information Get Into CCMS and Whole Person Care Summary?

A client record is created in CCMS when a client is listed within one of the CCMS record source databases. Once a CCMS record exists for a client, the CCMS database checks for matching records in source systems. Matching records are then attached to the CCMS record and client information is displayed on the Whole Person Care Summary.

  • Automatic Loads

    • Automatic copies from major systems occur nightly, with a possible 2 day lag:

      • Avatar

      • Invision/LCR (only selected vulnerable populations)

      • PES

  • Periodic Loads

    • Records from other systems are also loaded periodically (most monthly):

      • CHANGES (shelter)

      • EMS (rarely)

      • JIM (Jail Health)

      • MEDS (public benefits information)

      • Navigation Centers

      • SF Health Plan

      • Vital Records (Death Registry)

  • Direct Entries

    • Information is also added by people using CCMS directly, at any times they are working in CCMS:

      • DAH (Direct Access to Housing)

      • Medical Respite

      • SFHOT

      • Sobering center

      • Stabilization rooms
