  1. How do I access Whole Person Care Summary?

  1. Navigate to to access the DPH WebConnect Sign In page.

  2. Enter your Username and Password and click Sign In.

  3. If this your first time using DPH WebConnect, you will be prompted to set up two-factor authentication. Two-factor authentication enhances the security of your account. Click the Start setup button to begin account setup.

    If this is not your first time using DPH WebConnect, please proceed to step 4.

    a. Select the radio button for the type of device you are adding and then click Continue. This step is required for security and identification purposes. Please note that it is recommended that you add a mobile phone as your device.

    b. Once you have selected mobile phone as your device, you will need to install the Duo Mobile app by following the steps below. Please note that these instructions are for Apple iOS i. Launch the App Store ii. Search for "Duo Mobile" iii. Download the Duo Mobile app.

    c. After downloading the app on your mobile phone, click the "I have Duo Mobile installed" button to proceed with setup.

    d. Activate Duo Mobile on your mobile phone by following the steps below. Please note that these instructions are for Apple iOS.

    i. Open the Duo Mobile app on your mobile phone

    ii. Tap the "+" button

    iii. Scan the barcode on the computer with your mobile phone

    iv. Click Continue.

    e. The activated device will now appear under your settings and devices as your default device. You may also add another device or choose a default action upon logging in with your default device. If needed, you may also reactivate Duo Mobile for or change the name of your device(s).

    f. Choose the default authentication method that will appear when you log in with your default device. The recommended authentication method is for Duo Mobile to send a push notification to your mobile phone.

    You have now successfully set up two-factor authentication are are ready to access CCMS Patient Summary. Once your account has been set up, you will not be prompted to go through this process again.

  4. Your selected default authentication method will be sent. If Duo Mobile is your default authentication method, you will receive a push notification on your mobile phone. Accept the login request by tapping "Approve".

  5. Click the SF DPH CCMS Web Application Link.

  6. Enter your Username and Password and then click Login.
