Death Registry

If the client is recorded as deceased, a link to the Death Registry will appear in the Left-Navigation Links. Date of death from the Death Registry is the most accurate and will be indicated as "Confirmed by Death Registry" under the Client Identifiers section of the Home Page.

If the client was reported deceased by other sources, such as LCR, UCSF, or CCMS Main Application, the Home Page will list Date of Death as "Unconf by Death Reg."

Clicking on the Death Registry link for a client who has been confirmed to be deceased will display additional details about the client's death. The buttons shown at the top of the page can be used to help filter specific information to view.
